  • Y5 & Y6 - Health Related Behaviour Questionnaire (HRBQ)

    Tue 20 Jan 2026

Every two years we are invited, along with several others in the local area, to participate in the Health Related Behaviour Questionnaire (HRBQ) in January and February 2026.

We have been asked to support the work of the Public Health and Wellbeing Directorate at Dudley Metropolitan Council through taking part in this survey and stand to benefit substantially through our participation in it.

The children in Y5 and Y6 will complete the questionnaire in class, answering a variety of different questions. The questionnaire is completely ANONYMOUS and no-one can link the answers back to an individual child.

Pupils will be asked a wide range of age- related questions about their health behaviours. The topics the questionnaire covers includes:

    Personal background e.g. family dynamics

Risk taking (drugs, alcohol, tobacco and vaping)

    Emotional health and wellbeing

    Healthy eating

    Leisure

    Physical activity

    Relationships

    Safety

    School life

We hope you will agree with us that this is an exercise in which we wish to co-operate fully. We will assume you do approve unless we hear otherwise and we look forward to working with you in planning improved health promotion for our pupils.

As part of the survey, it will be helpful if your child brings the following information:

Their postcode (this will be used only for drawing maps of results in the area; your house will not be identified)

    Many thanks
